See If You Qualify
– 18 years or older
– Must have a valid Arizona Driver’s License or Arizona Identification Card
– Must have an Arizona residential address
– Must have at least ONE of the AZDHS Qualifying Conditions
STEP 2
Schedule An Appointment
Use the online booking system or call 844-466-5362 to schedule an appointment with a board-certified certifying physician. Gather your medical records from the last 12 months that state your qualifying condition. If you don’t have current records, don’t worry – we can help you establish them.
STEP 3
Visit Our Doctor
Visit our medical center for your appointment with one of our board-certified physicians. They will perform an evaluation and certify you on the day of your visit.
STEP 4
Receive Your Card
We file ALL state paperwork on your behalf for your convenience. Within 2-3 weeks, the Arizona Dept of Health Services will mail your new card directly to you. Congratulations!
